Complete this application to add an education site affiliation for classroom/lab and/or clinical training purposes in your training program. The Board must have knowledge of the affiliation by submission of this application and the signed affiliation agreement between both parties before the program places any students on site. NOTE: Healthcare facilities eligible to be a clinical education site are a hospital, licensed nursing facility, licensed residential care facility, or licensed assisted living facility per OAR 851-061-0090(2)(c) and (4)(c).
